At Northtowns Cardiology, we strive to provide high quality care in a comfortable and efficient environment.
Please arrive at least 15 minutes prior to your scheduled appointment. Your early arrival allows the provider to see you at your scheduled appointment time, and to complete any necessary paperwork beforehand. Failure to check in at the arrival time may require your appointment to be rescheduled.
We work with most major insurance companies including Medicare, Independent Health, Blue Cross/Blue Shield, Univera, United Health, Cigna, Fidelis, and Aetna.
We require all patients to show their insurance card and a photo ID while registering for their appointment. Failure to do so will may require your appointment to be rescheduled.
There is a $35 fee for returned check processing.
Additional fees may apply if more than one service is performed in the same day. This may depend on your insurance provider.
You are responsible for full payment of your copayment, deductible, or coinsurance at the time of service upon check-in. Failure to do so will result in your appointment being rescheduled. A $20 administrative fee will be billed to your account for co-pays that are provided at a later time.
There is a $40 no show fee for appointments not canceled with 24 hours advance notice.
Records requests will be assessed a fee of $0.75 per page. If records require mailing, patients will be obligated to pay that cost as well.
There is a $10 fee payable prior to the completion of any forms.
